This 2nd Answer to Writ of Garnishment form is used in conjunction with Form E above when there is a continuing lien on wages.
The difference between Form D and Form E is that Form D "Employer Form" includes instructions to employers on how much money can be legally withheld. Remember to check to make sure you are using the right form.
Whichever form is used (D, E or F), you'll need four (4) completed copies.
