This Writ of Garnishment form is called, Continuing Lien on Earnings, is used when another writ of garnishment against the debtor's earnings is already in place. This form will continue the lien for 60 days after the previous writ expires. writ of garnishment form C
Note 1: If the garnishment is against a bank, you must include a signed statement of the debtor's place of residence and business, occupation, trade or profession or his or her account number.
Note 2: If the writ is issued under a court order or judgment for child support, you must insert the following phrase into the caption: "This garnishment is based on a judgment or court order for child support."
Whether you use form B or C, you'll need four (4) completed copies.
